That is a strange one. I’m thinking there may be an issue with your actual WordPress database. Deleting Wordfence and all table data essentially starts you from scratch with Wordfence. If the problem returns when you reinstall, the problem is most likely outside of WF. Try checking the DB’s permissions. My colleague, Matt, suggested this doc:

How long is your block time rule set? The blocks will eventually expire. The default time is 5 minutes. Also, keep in mind attackers can change their IP addresses and then attempt with the same username again.
If they use the same name to login, from the same IP, within the block time you have set, then they will be blocked.

You should be able to get rid of file.php. It looks suspicious to me since I don’t have it on my test site. Run another scan after you remove.

Just to clarify, are you wanting this so you can easily see what site the alert is coming from? Are you running on multiple sites?

It sounds like there might be a problem writing to the database since IP’s aren’t being saved. You might want to have your hosting provider check the permissions and health of the database.
Is this a new install on your site or has it been working normally until now?

Do you have an email address set in WordPress under Settings->General?
The issue could be with how the server is configured to send mail. Here is a post that offers up some suggestions:

Thinking about this a little bit more…you could probably block these by using Block by Referer under Advanced Options.
