Hi,
Do you have any custom rules added to your .htaccess file or other plugins that write to .htaccess? Do you have Falcon caching enabled?

When settings do not save it usually indicates that AJAX is being blocked from wp-admin. Open up a javascript error console in a browser and see if there are errors when you try to save.

Usually, we see these kind of attacks from outdated themes and plugins. Make sure you remove any unused plugins or themes from your site. Update all themes and plugins that are in use. If you are on a shared hosting provider, you can be infected through other sites hosted on the same server. I’d recommend starting here:

Falcon does not currently include browser caching. You can use custom lines in .htaccess for this, if you like. Currently, Falcon and W3TC should not be used at the same time, as there are many possibilities of conflict when multiple caches are enabled. We’ve had similar requests and have it noted in our internal system.

Check the javascript console for errors when saving. Your site may have an issue with AJAX.

This could be a few things. Your hosting provider may be blocking the connection to our server or there could be a conflict in a theme or plugin you have installed. Try using the connectivity test in Wordfence options. It will let you know if your site can connect to our servers. If you’re not able to connect, you’ll want to check with your hosting company. Also, try the option to start scans remotely and see if that helps.

When did the formatting change? Was it after an update or an install of another plugin? If it was working correctly previously, then identifying when the change occurred can help track down the reason.

So you have Falcon enabled? How does the site behave if you disable Falcon? A 403 error could be the cause of incorrect permissions or possibly an issue with the httpdocs directory.

How are the permissions on your htaccess file configured? Is your htaccess file in a non-standard location?

Error 28 is a low disk space error. You’ll want to contact your host and have them look into it. When there is low/no disk space, all sorts of weird things can occur.

You should have plenty of memory now. Have you adjusted memory_limit in your PHP.ini file and restarted your web server as well?

Was the theme ever open source and available via the WordPress.org repository? We always recommend using themes and plugins that are actively updated and patched when new threats are discovered. It sounds to me like you know this. What would it take to update the site to a new and actively supported theme?

You can change the email address under Wordfence options. After you change the address, send a test email to confirm it is working.
