Check your PHP memory limit. This seems to be one of the common elements with the Internal Server 500 errors and this Jetpack update. You may need to increase the PHP memory limit.
Jetpack Comments doesn’t support CAPTCHA currently. Since it is loaded via an iFrame, plugins are also unable to modify that right now.
The bug with the Follow checkboxes will be fixed in the next release of Jetpack.

You need to increase the PHP memory limit. Looks like currently yours is set at 64MB. Try increasing that to 128MB.
@jmo@wordman.us: try clearing W3’s cache entirely and/or disabling it temporarily.
Are you running WAMP with a localhost address? If so, Jetpack is not currently designed to work on local installs.

What is your site URL? Has the domain been registered or updated within the last 72 hours?
Try changing the theme to a default one. Twenty Eleven would be best.

You could try adding the following to your functions.php in your theme:
remove_filter( 'the_excerpt', 'sharing_display', 19 );Be aware that this will remove sharing buttons from ALL excerpts on your site.
On my site, Jetpack Comments respects the setting in Settings -> Discussion marked as “Comment author must fill out name and e-mail.” This is what controls whether a user must fill out their name and email in core WordPress.
