If possible can you check with your hosting provider if “allow_url_fopen” this option is enabled or not? Because this could be the reason of the issue.

I checked and the CSS from your theme was conflicting. Please add below CSS code from plugin Settings >> Custom CSS

.acceptVoucherTerms input[type=checkbox] {
opacity: 1 !important;
}

If possible can you share your website URL? Because this looks like conflicting with theme CSS. I will give you CSS code that you can add from plugin settings and resolve this issue.
