In order to be good stewards of the WordPress community, and encourage innovation and progress, we feel it’s important to direct people to those official locations.
Forum volunteers are also not given access to commercial products, so they would not know why your commercial theme or plugin is not working properly. This is one other reason why volunteers forward you to the commercial product’s vendors. The vendors are responsible for supporting their commercial product.
All that said, this should help:

I use a plugin called Caldera Forms quite a lot. You’ll definitely be able to achieve the same look as the form in your screenshot.
With regard to the two column layout, you’ll need to work out how to do that with your theme.

Try deactivating all plugins. If that resolves the issue, reactivate each one individually until you find the cause.
If that does not resolve the issue, try switching to the default theme for your version of WordPress to rule-out a theme-specific issue (theme functions can interfere like plugins).
Please make sure you have a recent backup before you make any of these changes.

It looks like you’re using a commercial theme, so it would be best to approach the theme developers for support. From what I can see, it’s a crowd called Gravity.
The two links you’ve posted point to two different instances of your website, the one that works is the staging environment of the theme developer, the other is your actual site.
Forum volunteers are not given access to commercial products, so they would not know why your theme isn’t working properly.
