If you can tell me what versions of PHP, EBD, and Contact Form 7 you are using, that would help diagnose the issue.
The form on the page seems to be rendering properly and if this was a major DOMDocument problem it would error out instead of giving you a warning.
Is you WordPress in debug mode?

By default Contact Form 7 uses [your-name] and [your-email] for the users name, and email, so that is what EBD looks for for name and email. if you use those as your field names, and in your email templates, they will show up.

These issues are most likely do to the new version of Contact Form 7. They have deprecated some of the settings EBD was using to serve your links. The newest update of EBD, 5.1.0, should solve these issues.

It’s the spaces after the commas that is causing the issue. Remove those and it will work.

That very well may be the case. Email Before Download uses the template created with Contact Form 7.

This is an issue with older versions of PHP. In the previous update we changed our array declarations to be backwards compatible, but we must have missed a few.
5.0.8 will be released within the next hour and will fix this issue.

The forbidden domains field does not need the “@” to prefix the domain. Removing them will fix the issue.
We will, however, make sure to take them into account in the next update.

I’m sorry to hear it’s been frustrating with the new version. We needed to rewrite it to use the latest WP standards since it was originally written many years ago. We hoped it would be a seamless upgrade, and I think/hope it has been for many people, but we will keep trying to address items for people that upgraded and have issues.
Are you running 5.0.6 at this time?
