Ninja Forms isn’t currently designed to be network activated/deactivated via multi-site. It needs to be activated on each of your blog installs. If you used network activation, chances are that the proper databases weren’t created.
We hope to add support for this in the near future.
If you have activated each of the blogs individually and still have the problem, I might be free tomorrow (I live in GMT) to take a look at it.
The plugin stores the forms and fields inside the WordPress MySQL db. Removing the files and re-uploading them won’t affect those.
There was an error with our commit to the WordPress repo. If you download the plugin again and re-upload it, that will take care of the problem. I’m sorry for the inconvenience.
If you re-upload the plugin again, your forms should be fine as long as you didn’t delete anything in your MySQL database.

This is something that we have on our development track to fix and should be taken care of in a week or two. Sorry for any inconvenience.
We have had a user with what sounds like a similar issue on our wpninjas.com forums. I think that I have solved the problem and will be rolling the fix into Monday morning’s update. Please try out version 2.0.5 Monday and let me know if that takes care of the problem.
Thanks for the heads up! I agree, there are a ton of things we can add to make this plugin more awesome. We recently completed a re-write of another plugin, and that has kept us super busy. We plan on re-visiting Ninja Announcements in the near future. It will probably get a complete re-write as well.

I’m not really great at CSS, but if you send me a link to a page with the form you are trying to style, I’ll take a look at it.
I’d have to look at your wp-admin to see what’s really going on there. This is usually caused by the activation functions not running properly.
Version 2.0 or higher of Ninja Forms should take care of this issue. If you notice it again, please create a new thread for it.
Versions of Ninja Forms after 2.0 will fix this issue.
