This wouldn’t be coming from CloudFlare. I would also recommend actually setting a CloudFlare PageRule to exclude the wp-admin portion of the site from CloudFlare’s caching and features (we have seen conflicts arise when doing some things in the back end with performance and security features on).

I’m not sure I’m following this. If our data sources have new threats or spammers identified, then this will trigger the challenge behavior as well (this updates basically in real time).
Note: Make sure you only turn on Minify options at one service & not both. If you have CloudFlare’s minify turned on & at the plugin, you may see a conflict eventually.
Strange. You might want to check with the plugin author (generally see that plugin in source as well). Don’t have a fix, unfortunately, because those plugins aren’t written by us.
The W3TC source code (in my experience) would generally appear at the bottom of the page you’re checking source on. Are you sure the plugin is configured properly?
Note: It will be hard to look at without the actual domain in question.

That’s old content. There’s a more recent entry for W3TC. We’re also moving to a new system & that content will be coming out.
Are you using W3TC and CloudFlare? Yes, CloudFlare would work without it. Enabling CloudFlare in W3TC is actually a good thing because you don’t need to install the CloudFlare WordPress plugin as well.

Note: I would create a CloudFlare PageRule to exclude the wp-admin back end from CloudFlare.

I would probably recommend having the CloudFlare WordPress plugin installed.

No. Seems odd & probably difficult to troubleshoot this issue here (seems like a hosting provider issue). Any insights from your hosting provider about that?

Can you make sure you don’t have minify options turned on at both CloudFlare and W3TC? You only want to have one option active because conflicts can arise.
Did you change any of the static content on your site recently (css, javascript, images, etc.)?
