Thanks for the follow up @malawimama. Sounds like an awefully weird issue there. I would be willing to bet it was another security plugin (a lot of us use .htaccess for our “magic”).

…disabling and reenabling the plugin will rewrite the .htaccess rules which should fix it as long as all the references in your options table for the WordPress core are pointing to the correct place (check your general WordPress settings).

That can happen if you have problems with an existing site. Sorry to hear about your issues.
@knobby44 I’ve had problems with some plugins with the hard-coding issues but mostly it’s themes. Sometimes I feel like theme devs never get rid of links to anything they no longer use in their themes. At this point I have all my sites on either Genesis themes or Standard theme and the issues have almost completely disappeared.
add wp-content/themes/theme-name/cache to your exclude list and you should be able to exclude the whole directory. I’m wondering if the file always exists or if it comes and goes depending on state?
Thanks. Under single site it shows up as red because of the types of accounts many folks create for guest blogging, etc.

You get it with filter suspicious query strings? Sounds like a conflict with another plugin in that one. Are you using anything else that would fire at login/logout?
