Seems your issue related to incorrect URLs for css and js files on your server.
Please see screenshot: http://c2n.me/3mJcDmb.png. Based on provided link, your site is located on http://myfrenchcity.com, but you have a lot of URLs that linked to http://cdn.myfrenchcity.com. Our plugin linked to http://myfrenchcity.com, but contains this strange path of server file system – www/myfrenchcity_354/public/.Perhaps it can be related to site migration via some automatic tools or server settings. Could you please check this from your side, because we can’t do this from our side.
Regarding to API key, OpenWeatherMap API key allows to increase number of requests to OpenWeatherMap API for receiving weather data per hour. Our plugin allows to receive weather data without API key that works normal for site with average attendance.

“wordpress” part it is standard WordPress feature for mail. Currently, you are not able to change sender email & name via settings of our plugin. The most easy way to do this, it is install and configure some third-party plugin for SMTP email settings: https://wordpress.org/plugins/search.php?q=smtp.
We use this plugin – https://wordpress.org/plugins/postman-smtp/ for our
contact form on the demo site. More simpe plugins:

As described above, our plugin used standard WordPress wp_mail() function, that sometimes has issues with specific email addresses, i.e. not gmail etc and also it can be related to your host’s email policies. In this case you can try to setup some 3rd party plugin for SMTP mail settings.

Could you pls check your Spam folder. Sometimes email from WordPress marked as spam.
Also, our plugin used standard WordPress wp_mail() function, that sometimes has issues with specific email addresses, i.e. not gmail etc. In this case you can try to setup some 3rd party plugin for SMTP mail settings. It should help.

Currently your site is not available with 500 – Internal server error.
Unstable work of the server can be possible cause of your issue. You can try to check log files on you server, perhaps you find some usseful info for us there.
Regarding to notifications, it also can be related to described above and you also can find more info here.

And try to send a test email from our demo site to be sure that notifications are working – you should receive a letter from support@webcodin.com after the form submission.

Our plugin allows to create only one form with dynamic field set. Perhaps in the future we will add possibility to create different forms for this plugin or as separate one.
