Our plugin uses Colorbox JS library for lightbox functionality. Seems your theme or other plugin uses the same library and this can be cause of this issue – possible conflicts between style files.
In case if you know installed plugin with similar lightboxes – you can disable it to check this assumption, because we’re not able to debug this issue from our side – it is not common plugin issue and related to your personal site configuration.Forum: Plugins

After form submission should be automatically sent two autoresponder (“no-reply”) notifications on admin email address that defined in the plugin settings and on user email (in case if you properly send plugin settings). More information about autoresponder you can find in the plugin documentation.

It is so sadly that you are “disappointed” with our plugin. Just for your information, it is not a “shame” – it is described functionality of our plugin that suitable for over 10K users based on plugin statistic.
We always appreciate users feedbacks that related to existed plugin functionality and suggestions for future updates, but we a bit do not understand such feedbacks about functionality that plugin do not provide and also this functionality did not described in the plugin documentation and description (you can check it in the first paragraph of the plugin description).
In any way, if you don’t want to be “disappointed” and get a “shame” after installation, we can only suggest you to read plugins description more carefully for future.

Re-rechecked this issue again. For storing of user options our plugin uses custom table in the DB. This table should be created automatically after plugin installation. In the site DB this table – wp_wcp_useroptions. Based on errors from provided screenshot this table wasn’t created that can be related to restriction for creating of tables in the DB on server level. You can try to deactivate and activate plugin – in case if issue won’t be fexed – please try to check your server settings (possible something like disabled @@global.innodb_large_prefix variable (http://dev.mysql.com/doc/refman/5.7/en/innodb-parameters.html#sysvar_innodb_large_prefix)

In case if you have issues with blocked location field in the plugin settings, please go to the “Plugin” tab and disable “Google Maps API”.
This issue related to changes in the Google Maps API for new domains.
It is great that you gave us a low rate – however it won’t help to solve this issue faster, especially if you do not provide any useful information about your issue and use “Review” tab as “Support” tab.
All normal ways to have a support from our team are described in the plugin description.However, we’re working on this issue and hope that we will solve this issue soon.
