Yes, it seems like you are on the last full version,
Is the widget file inside a directory in the uploads/transposh/widgets (mind the s) dir? or is it just there, both should work, but I rarely tested the dirless method
Did you try copying a current widget there?
I read what you have written, and since I keep getting messages from there I run and tested the form myself – works for me, make sure that you are using a proper email in your from address and that the captcha is solved correctly

Try changing your default translation engine, you might have gotten blocked on one of them (normally temporary, but sometimes it is permanent, mainly on shared hosting providers)

First, you must be using the full version. Not supported on the wordpress.org version, but since I see 1.0.4.1 I think that you already have this,
Just make sure that the webserver user has permission for that directory, I suggest placing the code in a sub directory
You can test that its working by copying the current widgets to that directory, they should be listed with a (*) in the widget appearance selection box

(not that I think there is such information, but better safe than sorry)
But yes, the file should be automatically created.

I don’t think it has anything to do with this, you will need to inspect the calls that are going on, sometimes the translation proxy might stop responding (your site might get temporary black listed on the translation engine), You may try switching engines, and maybe enable transposh debug logging to find out what is going on.

Would appreciate removal of this post blaming me of robbery.
The code presented here does not exist on the wordpress.org version of the plugin. and is taken out of context, and anyhow – there are ways to criticize other people, which the author clearly ignored.
