It looks like you are loading jQuery 2.1.1 from Google instead of using the default WordPress jQuery. Do you mind loading WordPress’ default jQuery and see if the issue persists?

It’s possible that this is a conflict with your theme or another plugin. Do you mind trying to deactivate other plugins and change to the default theme (Twenty Seventeen) in order to see if the issue persists?
