I tried to create a work around in the meantime for most other private pages and to my surprise even the work around doesn’t work.
Work around malfunction:
To try and temporarily fix the issue until you find the bug I hid the Address box from the website (still there but hidden from view) and added a raw text box. To my surprise it still came up with a ‘something went wrong’ when clicking submit if you type the word United Kingdom or Zambia into the text box titled Travelling from where?. It works for United States or South Africa or misspelled words though. I’m sure there are issues with other country names, though I’m not going to try them all. Please help. This is materially affecting our business minute by minute.Existing Form with Address box malfunctioning:

Thanks again for the help.
No I did deactivate and completely removed the old SMTP plugin, so that shouldn’t be an issue. I have also gone over any other plugins and it doesn’t appear that we have any optimization plugins active that would affect this.

The plugin I was using didn’t seem to have the ability to not allow it to force the from name change. So I downloaded the plugin you recommended. Which we then set up connecting with our existing sendgrid account and linking with the api key. The test email worked and all set up seemed to be fine.
I turned off the force from email function box, and then tried out doing a test the form. The form did not send the email and had an error with the SMTP plugin stating that the email failed as the from name couldn’t be verified as matching.
So when I turn back on the force from email tick box then form will send through an email to our inbox. But then we are back to the same problem, with the email coming from me (our own email address) as well as our name. Is there something else I need to be doing for this to work?

Thanks for your help. So we seem to have sort of resolved this. We installed an SMTP plugin which is now ensuring we are receiving all email notifications.
Mailchimp is connected, and does seem to work. It will only send the subscription sign up list email the first time that email address fills out the form, not sending it on the subsequent form fill outs when that same email is used. Even if the person does not subscribe. We can figure that out with mailchimp later. Probably is fine to be set like this. The person doesn’t need to receive that mail multiple times.
But what is now happening since installing the SMTP plugin, it seems to be overriding the form settings and always having the form email coming from our own email address and name. We need to fix that, but I am not sure what I can change in the SMTP plugin settings without affecting the deliverability. I have reached out for help with this from the SMTP plugin developer forum.
