Sorry but I’m really not sure what the issue is here. You are being sent to the login page by content?
If you’ve updated a plugin, theme, etc than you should get a warning. If the files changes haven’t been touched than you might have a problem. Take a look at the files themselves to know for certain.
There is a bug that can send 2. Any more than that can be caused if you use database query caching.
Interesting. It isn’t failing in any of my test sites. Is it only one log that won’t clear or all of them (will the file change log clear)?

The filecheck, by definition, is memory intensive and disabling it should solve any memory issues that can’t be resolved by increasing the memory available to the application.
It sounds like your wp-config.php file is incorrectly closing php with the line
?>
Remove that line from wp-config.php and you will be fine.
Please make sure you are using 3.3 of Better WP Security as this issue should have been solved.
What is causing the lockout? Too many 404s or too many bad lockouts?
Wow! Sorry to hear that happened. This is something I haven’t seen anywhere else. Do you still have the error logs from that time so that I might look into what caused the issue? If so please email me at info [at] bit51 [dot] com.
The tinycms file and error_log should both probably be excluded in the settings if they are changing often.
As for the 404 on it, I would guess that the ultimate-tinymce developer left the link in the code somewhere but didn’t leave the file. I would suggest mentioning it to the developer of that plugin.
