fill out form ->Select package -> checkout

I want the user to give me some information with steps form (that has progress bar) and after that he will choose between 4 packages(1 month/3 month/6 month/12 month) and the last step of the form will be the checkout (I want it to be new checkout page, I have one in the website already and I don’t want to use the one I have).
