I would like to use this plugin in order to include URLs in post titles via the “Page Links To” plugin. Is this possible, and if so, how would I map this?

Never tried this, can you explain more in detail for better understanding? If possible try a sample import and post the results.

What I would like to do is to use Ultimate CSV Importer to (a) import a URL into Page Links To and (b) select whether to open that link in a new window.

The current version have no option to import for Page links plugin. We can only add this to upcoming feature list and can expect the support in future version.
