We are unsure what the issue is. Did you try delete those fields and Add them again? If that doesn’t work, provide your email for further communication.

What we have is a small box with the form, that appear on the right bottom corner, we call it FlipBox, and you have it under Addon tab of ChimpMate Settings page. Is it something you need? You can have some CSS work around to make it appear on left.
