We tested with GTM on our test site, and it did work nicely without any issues. Can you keep the plugin on, so that we can have a look.
Since the slider cannot be used, you may disable it individually from the ChimpMate Settings page.Thank you for your patience.

There is a Javascript plugin, jQuery.mmenu, working in your website, which provides a similar slider but with a menu(for mobile). This plugin interfere with ChimpMate’s code, which makes it unable to process as it should.
Can you please tell us what exactly is the issue with Google Tag Manager?

See if you get any errors( red text) in the Javascript Console of your browser. If any, comment them below.
In Chrome, Right Click on page => Inspect Element => ConsoleOtherwise provide your email ID for further communication.

Its actually all up to you, you may leave as it is, with only email field. Or if you wish, you may add other fields, like name, telephone, or gender or what ever to you MailChimp list, and then add them ChimpMate under Custom Form settings in General tab.

-Once you click the edit button for the form, below you have a list of fields, and there you should see a row for you groups, and across it there ‘ll be a delete icon. If you want to remove the groups, you can click it and hit confirm. To save the changes, hit OK button(below the rows of fields) and then hit Update Options.
This is how it should be working, and if it doesn’t provide your email address for further communication.-Let us have a look at your website, so that we get a better idea, what’s wrong with the font. Please mention your website URL.

ChimpMate only deals with the subscription forms. That part is all taken care from the MailChimp Backend.

-Check if you have any cache plugins installed, and clear the cache, to see any change. If that’s not the case, mention which theme, shows this glitch.
-You have an edit button across the form, in the General tab, where you can edit or delete the group options.

Yes of course every setting will be preserved. Even you may make a copy of your settings via Backup option in Advanced tab. However, as the Custom form option is not yet released in the Pro version, you’ll need to configure the API settings section only. We are in the finalizing stage of Custom Field/Multiple Form, and Multiple List for ChimpMate Pro, and will be released in 2-3 days.
