Make sure you are connected to MailChimp and not asking for for first name as required in MailChimp.

The easiest way at this point is to only include the code for SumoMe on the blog and don’t add it to your header for those other pages.

You are right.
If you want to have name you have to use custom form in advanced settings.
You can edit the look and feel in custom to make it similar.

I tested it and it is saving your appearance.
Click on the crown on the blue tab, then list builder then view stats and you can see that you’re kicking ass 🙂
Right now we aren’t supporting IE9 yet.
