This is odd behavior and not an issue with the csv or plugin. Is any way to access or look into our instance? Or a clone copy to test it in our testing environment.
@harwooje, this is permission issue. Kindly check wp-content/uploads/ultimate_importer is writable.

You can set the delimiters in Settings >> Features >> Export Delimiter and save it.
Can you add a sample of the same csv? did you tested it in our live demo ? If not give atry and post the results for further assistance.
@gbonati, we are working on this. Is this multisite? do you have any log details? curious to know the cause of the issues, so that we can plan for a release with a fix.

If you have any issues in upgrading kindly deactivate and install 3.6 as new install. This should work and give try, let us know the results. For older version you can get it here
We already upgraded in our live demo here http://demo.smackcoders.com/demowptwo/wp-admin/admin.php?page=wp-ultimate-csv-importer/index.php&__module=dashboard without any issues and no other users reported such error. Deactivate the plugin from admin and install 3.6 as fresh install. This should work. If not let us know with more details like log etc.
Version 3.6 with auto delimiter is released. Upgrade to get this issue resolved.

But, in the free version you can check the duplicates on the next import. Versioning, update features are not available for now in free version.
