Try to add check error logs if you still have issues.This feature with more advanced options is in pipeline target to release with 3.7 version. Media handling will be improved further from 3.7.
This is clearly permission issue. Kindly check the permission and add it if needed.

May be java script error, check your console for any error and browser for javascript enabled or not.
This is in pipeline. And we will be releasing our common lead capture plugin for all crm soon. The plugin is waiting for approval from wordpress.
