500 Internal Server Error (aka HTTP 500 error) is a generic error suggesting a problem with the website’s server, dont think because of plugin. Can you verify that first?
Yes, the provided image url is not accessible. Kindly ensure that image can be imported without any error like 404 etc.

1. Yes, you have to host images in external or internal urls. Dropbox images not supported.
2. No, file names are not changed unless it is a duplicate of existing one and this is wordpress default.
3. Feature image import have no issues when we tested it to reproduce this issue. Are you sure the image format and url is correct? It is downloadable form source? You can add a sample csv for any further support.Kindly try latest version 3.6.77

vii) After the investigation, PayPal declined your dispute on Tue, Feb 17, 2015 at 9:40 PM for trying to get money back after getting the product.We have nothing to do with this. If you have access to create helpdesk ticket then you should have access to downloads as well. We always support all our genuine customers, in your case we faced lot of false information with no proof submitted as you claim.
We should take this further legally the the next level to all channels with all proof, conversation history etc., we got against your claiming. Note that may have sensitive information’s also, if not responded.
