Your custom coded post type doesnt handle or registered as custom posts properly. Something odd is happening in your setup. Kindly ensure that you meet wordpress codex as mentioned here- https://codex.wordpress.org/Post_Types in particular with proper use of register_post_type
This may be because something is broken in your csv. The export is ok, just reopen the csv file again and check for format issues. Or else use google spread sheet, this will fix in most cases. Try with couple of post import, once you comfort you can prepare for final import.
ACF is not fully supported by free version, only features similar to wordpress custom fields are supported.

In reply to: Discontinuing Zoho CRM – how does it affect plug-in?No, you cant use or build a form without a CRM configured, otherwise all form submission will fail with error. The plugin wont store any leads in wordpress. You have to find other solution for your need.

This feature not yet available but in pipeline along with alt and description import
You can simple export the pages using export option within the plugin. Now you create cloning copies of exported csv rows to make your modifications before importing it..
This is generally because of csv upload folder permission. Kindly chmod your wp-content/uploads/ultimate_importer directory to fix this.
You can use the duplicate check options to reimport posts that are not imported in previous import. Skip the graph thing as it not important.
Kindly share more details to identify the issue, also ensure permissions for your uploads and sub directories.
