What plugin is used for custom fields? Default wp custom fields and CPT UI fields should work without issues.

Yes, this is default by wordpress itself. Also you can override by mapping a csv column to slug field attribute. Kindly review our plugin, this will help us a lot.
This may happen when the import job is skipped before complete until the success message triggers. Other than that partial imports are not calculated as successful import.

Yes, it is possible. Get the right CSV sample files here as reference. There is no need to create category manually unless you need to import category slugs and descriptions. The categories mentioned in csv that doesn’t exists will be created on the fly.
Cannot reproduce, if you still have issues, you can rename the plugin folder which will be deactivated automatically
