I fixed the shortcode visibility problem on all sites. It was a JS error with Elegant Themes Divi upgrades that corrupted Google Analytics code with an ET update. Replacing the Google JS code in each fixed the error. It WAS NOT a Ninja Forms issue.
The submissions CSV missing and badly parsed data is the only outstanding issue I see with the new release (v3.0.26) and understand only some users have reported it. Will try to post back what I find out from NF in case it helps others.

Shortcodes stopped working on sites I manage after update as well.
