We are currently running Organize Series version 2.4. We first started using it in Sept 2011 so I think we may have done at least one update to it (possibly within the last month).
We’re running WordPress version 2.4 (again I believe may have been updated within the last month).
It seems to have only started happening recently so is probably related to updating WordPress / the plug in.
