thanks for looking at this issue so quickly
I have updated to 2.6.4
The problem still occurs when the check box field is mandatory.
The dialog box says : xxx is required even if the check box is checked
The same problem occurs with the check box which is generated when the “Enable Terms & conditions” is enabled in the Theme as this is also mandatory

I’ve just recently installed the plugin
It worked initially
then next day it failed with the same log messages as above
