1 – You can ignore. The focus keyword not in first paragraph warning is thrown out if there is an HTML element, code, image, or whitespace around the keyword which is affecting Yoast’s ability to scan for it.
2 – The keyword density for your post has to be between 0.5 and 2.5%. Please refer to this article: https://yoast.com/keyword-density-post-panda/
3 – Below is your meta description. Try removing “the” and see if the plugin recognizes it.

Hey Joe,
Does the issue go away if you deactivate Yoast SEO? If so, it could be caused by a conflict with your other plugins or your theme. Please perform a conflict check following the steps here: https://kb.yoast.com/kb/how-to-check-for-plugin-conflicts/Polylang can best address this concern. Please refer to their forum here.
Please get in touch with your theme developer on this. Breadcrumbs are added by the theme and not by Yoast. Although our plugin has a feature in SEO->Advanced->Breadcrumbs to enable customization of the breadcrumbs, those breadcrumbs need to be on the site beforehand.
Regarding your sitemap redirecting to the homepage, can you please flush your permalinks by going to Admin > Settings > Permalinks and save the page without changing anything? Once done, try loading your sitemap again and let us know what happens.
As for your other concern, by default, the Yoast SEO plugin creates sitemaps on the fly for the content within WordPress only. To add external sitemap to index, please refer to this kb article: https://kb.yoast.com/kb/add-external-sitemap-to-index/
