It seems there may be an issue with how the blocks are parsing the shortcode, or it might be a configuration problem. Shortcode conflicts can occur due to incompatibilities with the block editor or incorrect placement within your content.
Are you using WordPress default block editor or another plugin for that? And, when you added the shortcode, did it automatically take it as shortcode or were there any error messages or unexpected behavior?

Could you please share the screenshot of the tag you are comparing there? I see this GA4 tag (G-6XQSW3EENS) loading from your site currently. https://a.supportally.com/i/cXDaHV
If you are seeing multiple data streams here, then that would explain the different GA4 tags (which is totally fine). https://a.supportally.com/i/jtJbNz
Just a side but related note
To clarify, MonsterInsights will create a new data stream if your site’s URL doesn’t match the URL in your Google Analytics profile when running the setup wizard. If there’s an exact match (for example, whether your URL starts with ‘www’ or not), MonsterInsights will use the existing data stream.In your case, since a new data stream is being created and the GA4 tag differs, it’s likely that the URL in your existing data stream in Google Analytics doesn’t exactly match your site’s URL. Once you correct the URL in your Google Analytics data stream to match your website exactly—including ‘http://‘ or ‘https://‘, and the presence or absence of ‘www’ —MonsterInsights should recognize the existing data stream and not create a new one.
It’s important to ensure that the URLs match precisely because this is how Google connects your site with its services. Differences like using ‘http’ instead of ‘https’ or variations in ‘www’ can prevent proper recognition. So, make sure your site’s URL in Google Analytics matches exactly what is used on your website.
Additionally, one property in Google Analytics can have multiple data streams, allowing you to connect different apps and websites within a single Analytics property. This means having multiple data streams isn’t an issue and won’t affect tracking in any way.
However, if you have two separate properties for the same site, you’ll want to keep the one with the most historical data. When running the setup wizard in MonsterInsights, select the correct property during authentication with Google to ensure it’s connected properly.
By aligning your site’s URL across MonsterInsights and Google Analytics, you should resolve the issue without needing to manually edit code on every page of your site.

I am afraid the survey’s cannot be embedded outside the site currently. However, we do have a feature request active for this and based on priorities, developers are working to get this added within the UserFeedback.
For now, a part of your request can be achieved by embedding the survey on a specific page, and then email users to land on that specific page. Hope this helps.Have a great day!
