PHP 5.4.4The problems are happening to both the local and production env–they’re copies of one another. We’ve kept prod. on version 5.4.1.1, since that seems to work.
The local environment is where I test the upgrades. Last night, I went through each update to see where it broke and it breaks at version 5.5 and above.
After further testing, it appears that the problem crops up at version 5.5 and up. Version 5.4.4 and below seem to work as expected.
The current live site is visible here and is still running version 5.4.1.1:

The upgraded version’s running in a local environment so I can’t share the link, unfortunately.

Thanks for the tip caimin_nwl, I tried that and it’s still displaying “not found”.
Yes, I’ve tried deactivating/activating as well as uninstalling and re-installing. I suspect it’s conflicting with something in their site; I’m guessing in the database, since it doesn’t seem to be caused by any plugin/theme conflicts.

Here are the steps I used to get this working:
1. Log into your OpenX account and navigate to the Inventory Tab.
2. Click on “Zones” in the left menu.
3. Select the website you want to host the ads to then add a new zone.
4. After creating the zone, or if you already have a list of created zones, locate the zone id by hovering over the zone’s link and a link path should appear in the bottom left of your browser. The last bit of the link should read “zoneid=12345678”. This is the zone’s id. There may be an easier way of getting the zone ID but I couldn’t find one and this worked for me.
5. Go back to your WordPress dashboard and install the openX wordpress Widget.
6. Add the widget to the sidebar and enter the zone ID you obtained in step 4. Each widget instance should have a unique zone ID.Save and refresh your website’s page.

Resolved!
As agelonwl suggested, checked out the Settings > Formatting > Events section and swapped #_EXCERPT with #_NOTES in the ‘Default event list format’ field. Works like a charm.
