So you know I am not using this plugin to redirect anything and I am just using it as reporting 404 tool.
Here is the information:
PHP Version 7.1.18
This is just a sample from the .htaccess

I had the same issue, I did not use a code. If you have Cloudflare this will work.
On Cloudflare go to “Page Rules” > Create Page Rule >
First URL setting:
https://yourdomain.com/wp-admin/options.php
Then Add a Setting:
“Forwarding URL”
Select Status Code:
301 – Permanet Redirect
Then Enter Destination URL:
This would be your Home page or what ever page you want:
Make sure you flush your cache when done

Thanks for the information.
I checked the wp-config.php files and debug is off.
I am also using the Sucuri security plugin and the server “Environment Variables” settings is saying that “WordPress debug is not active”.
Your plugin works, just the “Server Info” is showing these errors.
I have looked everywhere to see where wp_debug might be enabled but I not found it yet.

I am a big fan of this plugin the way it’s, please do not do this.@rzlatic
They are locking threads deleting comments that are not favorable to them etc.
You will have to go manually and clear the logs via phpMyAdmin
