Hey @loai96ms
Thank you for alerting us about this.
It looks like you are using the Pro version of our plugin. I’m afraid we can only support the free “Lite” plugin via WordPress forums.
For support with Pro features, could you please open a ticket in our helpdesk here: https://formidableforms.com/new-topic/

Thank you for confirming this. Could you please export your form as an XML file and attach it to your response? If you need help with exporting your form, here are some instructions that may help.

Thank you for reaching out about this today.
I’ve seen this behavior before, usually, is related to a setting on a SMTP plugin. Could you confirm if you are using a SMTP plugin for email deliver on your website? If so, could you share which one?
Best,
Thank you for the screenshot. To confirm the site is running the latest Formidable Forms version? The field doesn’t look like our standard and current Checkbox and Radio field options.
Best,
Hey @kimchitoast ,

You can assign the value of the step and the range on each slider field options. For example, The step value could be 1, and the range could be from 0 to 20.
Using the field calculation feature, you can sum the values of all the slider fields together into a total.
You can also create that summary using our built-in features. Please take a look at the following form templates:

Yes, this is possible with Formidable Forms. You can set up the weekly and monthly plan based on the user selection using several collect payments actions and conditional logic. Both our Stripe and PayPal add-on support subscriptions.
