Thanks again for responding.
I deleted & reinstalled CF7. Same results in the widget.
If you look at this page, http://www.sleepapneadrhair.com/contact-us/
You will see that while the widget can’t seem to find to find CF7, when I paste the same shortcode into the visual editor for the page, you can find CF7 under the map. I also placed Black Studio Ice TinyMCE in the right sidebar – I have labeled them for your benefit – if you don’t mind looking again.
Why would the visual editor for the page content be able to find CF7, but the widgets can’t?

I had planned on getting the Appointment Request up on the site first, and then modifying it.
