    Frank,

    I’ve found the issue to be a result of another plugin being installed. The plugin name causing the issue is “SP FAQ”.

    Once I deactivated SP FAQ, the quiz plugin works as it should.

    Here is some further info. I had a WP site running 3.5.2 and installed the plugin. No issues…I was able to create a quiz, add questions, and none of the blank screen troubles. I then update that site to WP 3.8.1, and the plugin still works as it should.

    Could there be an issue of a new issue on WP 3.8.1?? Any suggestions?

    I noticed another strange occurrence of the blank page happening on the Quiz Options page as well. After I go to the Quiz Options page if I mouseover the top wordpress bar on either the Comments, New or WP symbol icons the entire page goes blank just like in the other cases? Very odd. I’ve also tried this using IE 11 and it does the same thing.

    – The Add Question form disappears before I can enter anything
    – The entire browser window goes blank (Chrome)
    – The ? does display the Help Popup (however I noticed if I just mouseover the X (close) on the Help popup, the screen goes blank just like the above
    – have not tried other browser yet
    – WP 3.8.1
    – Win 7
