Thanks for writing in and we’re sorry to hear about the trouble you’re facing.
We’ve seen cases where the minify and combine script feature of caching plugins causes WPForms script to break which results in issues like you’re getting right now.
When you get the chance can you please try the following?
- Exclude the page or post where the forms are from JS optimization. Please check here for more details.
- Exclude this script from minification:
/wp-content/plugins/wpforms-lite/assets/js/wpforms.js
Kindly make sure to clear your site’s cache after doing any of the changes above.
And in case you are not sure where to set those changes, it would be best to get in touch with the LiteSpeed Cache support as they should be able to provide you more information.

Thanks for writing in and we’re sorry to hear about the trouble you’re facing.
Since you do not have a caching plugin on the site or anything the minifies the scripts which are the common cause of this issue, I would like to check your site and see if there’s anything I can find.

I have just checked your site on a mobile device and I was able to view the images in the image choices field for both the portrait and landscape view. Please check this screencast for reference.
I noticed that you are using a caching plugin on your site which might also be what’s causing the images to load intermittently so when you get the chance, kindly check on your caching plugin’s settings.
Lastly, it looks like you are using a paid version of WPForms. Please note that with an active license subscription, you have access to our email support. So, please drop us a line through our contact page.

When you get the chance, can you please check in your settings and make sure that the GDPR related settings are enabled?
Once that’s enabled, please edit your form and make sure that you have added the GDPR Agreement field in your form.

It looks like the specific issue you’re having is related to PHP 8, so unfortunately you will need to have your server set to run PHP 7.x. At the moment, we are not quite ready with PHP 8 compatibility, but it is coming soon on the horizon.
I believe PHP 8 support is coming in the next major release, but that is subject to change based on a number of factors. When you have a moment, could you ask your web host to set your site to run PHP 7.x then test the site again? I believe that should resolve your issue.

I’m sorry to hear that you’re having issues with email delivery.
To troubleshoot the issue, you can check that your form notification settings are configured correctly by following our tutorial.
If your notification settings are correct, the issue is likely because the emails sent from WordPress are not authenticated (unlike emails sent from dedicated email service providers). As a result, they are likely to be filtered by the receiving mail server.
To resolve email delivery issues like this, you can configure a SMTP plugin on your site so that emails are sent as authenticated. We have an article with a list of recommended SMTP options and links to their setup tutorials.
