Do you happen to have a Snippet plugin active on your site?
If so, it might be worth checking if you added the code through the said plugin as that is one of the ways we suggest users to add some custom snippets on their site.

It looks like there could be a styling conflict between the site theme and the forms. To fix this, please try the following custom CSS snippet:
.wpforms-field input[type='checkbox']:before, .wpforms-field input[type='checkbox']:after { display: none; }And in case it helps, here’s a tutorial from WPBeginner on how to add custom CSS like this to your site.

When you get the chance, can you please try to set the Include Form Styling setting of WPForms to Base and form theme styling and see if it fixes the issue?

I’m sorry to hear that you’re having issues with email delivery.
To troubleshoot the issue, you can check that your form notification settings are configured correctly by following our tutorial.
If your notification settings are correct, the issue is likely because the emails sent from WordPress are not authenticated (unlike emails sent from dedicated email service providers) and not because of the browser used to send the form. As a result, they are likely to be filtered by the receiving mail server.
To resolve email delivery issues like this, you can configure a SMTP plugin on your site so that emails are sent as authenticated. We have an article with a list of recommended SMTP options and links to their setup tutorials.
Also, it looks like you are using a paid version of WPForms so please feel free to drop us a line through our contact page.
From there, our support team will be able to take a closer look at your request.
