I haved the same problem, but i was able to fix this after i go to my application > Open graph and select the publish action, from there i checked “User Messages: Allow users to write a personalized message attached to this action.” and save the settings.

I have the same problem here, he worked like 1 week and now has crashed and i didn’t make changes to the blog.
I you use “theme my login” and “register plus” with captcha activated, you must put the code for captcha from wp_login.php <?php session_start(); ?> also in theme-my-login.php, u can find it in /wp-content/plugins/theme-my-login/
