In the Admin panel, go to Settings > WP Customer Reviews
Find the Business Information section, and change the Enable Business hCard output to Never and the Business hCard Visibility to Hide HCard to Visitors.If that does not fix it, then try changing your theme to a basic WP theme and check again. If it does fix it, you know it’s an issue with the theme. I suspect the first suggestion will fix your issue.

In reply to: [WP Customer Reviews] Not displayed, not stored the official answer@damahno adn @akalwp, what themes are you using, and did you try changing the theme to a basic WP theme and retest that function? 90% of the time the theme is causing a conflict.
PLEASE NOTE version 3.0 is currently being beta tested and is due to be released in a week or two. The developer is currently debugging.
@fionavar, if you are on a Cpanel hosting account (shared or reseller) on GoDaddy, you can log in to Cpanel, go to the Software/Services section, click on Select PHP Version, and change to 5.5.
Upgrading to 5.5 resolved for me too. Thanks to @moshiurmix for the fix.
