To remove the German translation, please download the zip file
extract the contents
goto woocommerece-checkout-manager > languages
find and delete both woocommerce-checkout-manager_de_DE.po and
woocommerce-checkout-manager_de_DE.mo files.and the upload everything to your server folder.

I have just tested the free version, and the information does show in the E-mail. Have you tried entering a name in the abbreviation section?

sod-woocommerce-checkout-fields.php is from another plugin,
this is WooCommerce Checkout Manager support. I would can’t say what is the problem.
