It only does manual (on demand) scans. Our company (Sucuri) actually charges extra for scheduled scans.

Yes, it happened because we changed the way the last logins are stored. So it is empty when you update the plugin.
After a few days (or if you try to logout and log back in), the table will be repopulated.

The plugin should not affect your WordPress language.
Maybe you also updated WordPress and chose the english version? Can you try again with 1.4.7 to see if it still happens?

Yes, other people had problems with it too.
We rewrote the code and tested it on many sites, so it should be working properly now. Please just update it again and let us know.
*you will lose all old login entries but it will start to populate itself automatically from now on.
