The plugin will work on a multisite installation, but it has no multisite functionality.

Looking at the error message and the code it would seem for some reason the default settings are not being loaded.
Can you give me some information about you setup.

os.There must be something different to out development and live system that it does not like.

I think I get the gist of your problem. There is no option to add a submit button, because its not optional. If you look at the second to last field on the edit screen you will see the ‘Submit’ button is there. The only thing you can modify about this is the prompt, and it will always be placed as the last visible field on the form.
Let me know if this is not the case, then we will have to do some fault finding for your system.

If your options are not already black then it but be the theme which is changing them.

It is unknown when the next version is released, due to heavy work load. It is being worked on when time permits.
The above problem should not prevent you from downloading saving or downloading results, just as long as you don’t edit the default data collection settings.
