The latest version displays the Add to Quote button on the single product page correctly.
You can identify if the problem is with your theme by switching the theme to a default WordPress theme.
If the button is not displayed with a default WordPress theme, you can check if there is a conflict with another plugin. Try deactivating all plugins except WooCommerce and YITH Request a Quote for WooCommerce.Forum: Plugins

Regarding the warning message, I made a test in a local installation and I didn’t get this message.
Are you using the latest version of the plugin and WooCommerce?
What version of PHP does your server have?Maybe there is a conflict with another plugin. Try to deactivate all plugins except WooCommerce and YITH Request a Quote for WooCommerce.
Also, please check if the products added to the quote have a price set.About the second question, you need a Child Theme and override the email template, located in:

the plugin is designed to work with WooCommerce. If your theme supports WooCommerce then it will work with no integration needed.

Probably there is a JavaScript conflict with your theme or some plugin. Have you tried to use a default WordPress theme and deactivate all plugins except WooCommerce and YITH WooCommerce Popup and check again?
