You mean only the UM registration form?
This is not a UM issue. Ask your host to disable the ‘PHP open_basedir’ protection, or if you can. do it yourself in your cPanel.

OK, it makes no difference that it’s a third party “free” plugin. Customization questions about other WordPress plugins are, for the most part, never discussed on this free UM support board. Please hire a developer to help you with your customization needs.
This is the support board for Ultimate Member free plugin for WordPress. Don’t wait for an UM update because that’s a css file issue with that addon. If this is a paid extension, support is provided here:

What does this have to do with Ultimate Member?
If you reinstalled “UM”, then it is the same as a complete flush, similar to clearing your cache, only this time you cleared specific plugin data.

Did you really do a theme conflict test?
Also, by the look your page code, you are using minimization. If you have some optimization plugin to minimize css and js files, take it off. If you have some Cache plugin delete all caches. That in itself can be related to your problem. Also I’d recommend you disable other plugins. This still may be a plugin conflict.
First check Admin panel => Ultimate Member => Set Up and see if the Login page can be changed to the page you want.
Otherwise, check Admin panel => Ultimate Member => User Roles => Your Members Default => Edit and see where the login options redirection is taking you to and modify url if necessary.

It looks as if Ultimate Member doesn’t support multiple custom profile forms that allow editing.The “Edit” option under the gear always funnels to one instance of profile edit.
