Turned out to be the Atahualpa theme. Funny, it worked all these years until the last update.

Sorry about the dual post.
This configuration worked in WordPress 3.1.3, Contact Form 7 2.4.5 and Atahualpa Theme 3.5.3

Problem fixed. Strange though.
In the drop-down menu items there were 3 spaces between a date and time. Replacing the spaces with an “@” symbol fixed the problem.
Evidently, you cannot have 3 spaces in a row in a drop-down menu.
