I think this is wordpress and not the plugin,. I will look further into this …
yes I can confirm this happens when an image with the same name is already in the wp-content/uploads folder. I had to delete them first and then they rename normally.

I also had this, to fix it I found I had to setup the config in wordpress form, otherwise if using wp-config.php it does not work for me if I try to disable it in that. It might be something to do with the server not been setup right for cron?.
