Look at who the author is on the post. Then look at the user’s profile account. Within the profile, check to see if they have the wrong profile URL entered in the Profile field there.

When you check the media item, in the media library, for the item that is getting it’s attachment page indexed… is it ALSO attached to an article?
It’s important to understand attachment pages and their ATTACHMENT to an article… which is two different things that sound confusingly alike.
Every media item on your site has an attachment page… that’s due to how WordPress stores media.
Each media item can ALSO be “attached” to an article (post/page/etc) by having originally been uploaded from within there.
Even though you can use the same image/media in multiple articles… the media can only be attached to one of them.
And if you upload the media to the media library, unless you manually create an attachment, this item is NOT attached to an article, but still has an attachment page.
This means that many of your media items likely still have attachment pages that can not be redirected to an article as the item is not actually “attached” to an article. In that case, those attachments pages can’t be directed anywhere.

Your focus keyword has a comma in it that is not in the description. Make sure an exact match is used and it should then be found.

You’ve made changes within the ‘after’ content… filling in data. However, it seems unlikely that you told Facebook to re-scrape the data. Facebook uses extensive caching and that’s just part of how they do things.

Working through that guide, when you get to part “B” after you fetch the URL in question… you will then see a (relatively small) “Scrape Again” button. Do that and see if things straighten out.
If they don’t, continue working through that guide.
What particular error message do you receive?
The filter that you are attempting to use applies to the main URL only (or some variation of it). There’s another filter for posts/pages. Currently, there is not a filter that adjusts taxonomies like categories or tags.
The priority of categories is based on the number of posts in that category. The more posts assigned to the category, the higher the priority. Publishing more into a category is going to push this number up for you. This allows the plugin to dynamically respond to changes in your publishing patterns over time and for Google to be pointed to topics that you are spending a lot of time on.

Additionally, you may want to check under the plugin settings, “Titles & Metas” on the “Posts” section and be sure you did not put a checkmark in the “hide” box.
