If you want to replace built-in lead form with a custom form, the plugin doesn’t provide an API for it. It requires modification of the plugin.

The plugin can group different media items into the lightbox. If your gallery plugin doesn’t support ability to create this type of gallery then it is possible to add items one by one.

The plugin doesn’t provide integration with Google Sheet. Free version supports integration with MailChimp and MailerLite. PRO version adds integration with Zapier service.
The described problem can be with a 3rd party extension. In any case it requires investigation and it is a pity you posted a negative review without a support request.

Free version doesn’t save quiz results and data of lead form. This feature is available in PRO.
Email in free version is only used for integration with 3rd party services (MailChimp, MailerLite and etc.)
We are constantly check compatibility with latest WordPress versions. The plugin is compatible with WordPress 5.6 (currently this is the latest version).

Sorry, but currently the plugin doesn’t support ability to define default page in a PDF document. We will add this ability for PDFJS viewer in a future version of the plugin.
