the Alter table error probably is an error of dbDeta class. For the my account page you can try to regenerate permalinks.
Go to Settings > Permalinks, and click save changes.

you can try to disable the plugin by FTP. You can do this simple rename or delete the plugin directory. I haven’t this issue on my installation, you can try to reupload the plugin.

these bugs have been fixed in the latest release (version 1.2.1).
Try to reupload the plugin by FTP, I think you solve all issues.
