First of all, my sincerest apologies for the issue.
I think you are running v1.4 of the plugin. This bug was caused by an older PHP version and how it treats a certain function. We have fixed the issue in 1.4.1.
Please delete the plugin folder (insert-headers-and-footers) from your website’s wp-content/plugins folder and then install the latest version of the plugin (v1.4.1) from: https://downloads.wordpress.org/plugin/insert-headers-and-footers.zip

Site is back up now. I tried Version 1.3.3, Version 1.4 and Version 1.4.1
None of them worked – all crashed the site. So I uninstalled it via filezilla. Uploaded fresh copies, deleted, updated, deleted, updated, etc. etc.
But the site was down and kept giving the fatal error message.
Restored my site to yesterday. Still no luck. Cleared cached again. Still no luck.
Finally, left it for a couple of hours and the site came back up. But I cannot install the plugin.
